Oak Leaf Care Club is NOT an insurance. Oak Leaf Care Club is a membership based program offering dental benefits and discounts on dental treatment rendered at Oak Leaf Dental. There are NO Maximums, NO Deductibles, and NO Waiting Periods. Membership fees are $320.00 per year for adult patients, $300.00 per year for each additional family member and $200.00 for children under 15 years old.
